Care Management

Centro Geriatrics helps take the task of care management away from families, so that the time spent with an aging parent or relative can be enjoyed stress-free. Care management is an important part of ensuring that a senior is living safely and getting the best care possible. 

We take a holistic approach to care management by looking at physical and emotional health needs, social and recreational needs, support systems and housing. We may liaise with other health professionals to help with the assessment including nurses, dieticians, occupational therapists, physiotherapists, home care services and other community organizations. Communication is important to good care. We connect with families, caregivers, and physicians on a regular basis and continually evaluate the care plan.

Our services can often be submitted to insurance benefit plans, making it easier for clients to access the support they need without financial strain.
